Job Details
- Location: Puyallup, WA
- Duration: 13 weeks
- Start Date: April 28, 2025
- Shift: 8-hour days, Monday to Friday with rotating weekends
- Weekly Hours: 40 hours per week
- Estimated Weekly Salary: $2490 - $2670
- Licensed Independent Clinical Social Worker (LICSW)
- Proficiency in EPIC software
- Minimum of 1 year of experience in case management
- Provide comprehensive case management services to patients and families
- Collaborate with healthcare teams to develop and implement care plans
- Advocate for patient needs and coordinate with community resources
- Maintain accurate and timely documentation of patient interactions
