We are seeking a dedicated Social Worker with experience in mom/baby case management to join our healthcare team in Palo Alto, CA. This travel position offers an exciting opportunity to work in a dynamic environment while making a meaningful impact on families' lives.
Job Details
- Location: Palo Alto, CA
- Duration: 13-week assignment
- Shift: 8-hour days, Monday to Friday, with 2 weekend shifts per assignment
- Weekly Hours: 40 hours per week
- Estimated Weekly Salary: $2830 - $3040
- Master of Social Work (MSW) degree
- Experience in mom/baby case management
- Willingness to travel to Sequoia Clinic as needed
- At least 1 year of relevant experience in healthcare jobs
- Provide case management services to mothers and babies
- Collaborate with healthcare professionals to ensure comprehensive clinical support
- Travel to Sequoia Clinic 1-2 times a week as required
- Maintain accurate and timely documentation of patient interactions
