At our Client' s Case Management Department, you will join a team of dedicated professionals engaged in the rewarding work of providing compassionate care to a diverse population . We are currently seek a qualified LCSW to join their staff as a Social Work Manager.
In this role, you will work with a multi-disciplinary team to support safe and effective discharges from the hospital, assist with the overall day-to-day responsibilities for directing and managing the activities of the Case Management Teams.

Job Description
We Require
- Minimum of 3 years experience in leadership, development clinical and staff development.
- Case management experience; hospital-based experience is preferred.
- Current NYS Licensure LCSW, Masters in Social Work is essential
- Ability to multi-task and effectively problem-solve
- Excellent English communication skills; Bilingual Spanish is preferred.
- COVID vaccine and booster is critical
