Job Requirements:
- 3 years of Nursing or Social Work Experience
- Inpatient - Acute Case Management/Care Coordination/Discharge Planning experience
- NJ Licensed or Compact RN or NJ Licensed Licensed Social Worker/LSW or LCSW
